***Please check with your insurance carrier. There are no guarantees for payment for services. You are 100 % responsible for your payment on your account. If you have a copay or coinsurance, you are responsible for that at the time of services.***
You are required to put a card in file to assist with ensuring that your scheduled time is secured
Emergency Telephone therapy (30 minutes/insurance does not cover this) $50
Weekend hours are available upon request; however, are at an additional cost
Intake appointments/Evaluations are 60-90 minutes and require paperwork to be completed prior to beginning the session before you can see the therapist
Although we accept insurance there are reasons that we may not be able to depending on the requirements of the insurance company and your therapeutic needs
Late policy: If you are going to be late, you must know that this is your time; however, it will not be extended beyond the set appointment time.
If you are later than 15 minutes late, it will be considered a no show and you will be responsible for the cost of the session (insurance does not cover this)
A fee will be charged for copies of any records requested by clients or representatives of the client (pending release of information from client)
Services may be covered in full or in part by your health insurance or employee benefits plan. It is the responsibility of the patient to check coverage limits, co-pays, and benefits. Please check your coverage.

Because we are not a 24 hour service, If you have a physical or mental health emergency call 9-1-1. If you believe that you are a threat to yourself (thinking, planning, considering hurting yourself) or others, please call either 9-1-1 or the nearest emergency service phone number (usually connected to the community service board).
All fees are expected to be paid in full prior to each session. If there is a late, no-show or cancellation fee on the account, it must be paid prior to scheduling your next appointment
Please note that your account is subject to 3% interest per month or 18% interest per year as well as fees incurred in the recouping process (to include court costs and/or attorney fees of 33% or $50 - whichever is greater in the event that you incur a balance
If you are receiving VASAP services you will be responsible for paying for services upfront or may be provided the option to make 3 separate payments.
If you do not call and cancel prior to 24 hours of your appointment or do not show for your appointment, you will be required to pay $60 or the cost of the session (Insurance does not cover this fee) depending on if the slot was able to be covered. If you no show for your appointment, you will be charged the amount for the entire session. Please be mindful that have reserved our professional time for you. If you miss or cancel three appointments consecutively, it is the policy of this practice to terminate treatment if applicable. If you miss a scheduled intake appointment three times you will not be able to engage in treatment no less than 30 days after your last missed appointment.
